Connected Care Plans

Find a plan that works best for you and your family

As a benefit-eligible employee with Intel, you can choose one of two Connected Care plans with Presbyterian Healthcare Services: Intel Connected Care High Deductible Health Plan (HDHP) or Intel Connected Care Copay.   

 

With both plan options you will receive comprehensive coverage through the Presbyterian Patient Centered Medical Home and the Connected Care network of providers. Both plans include in-network and out-of-network benefits, cover preventive health services at 100%, and include comprehensive drug benefits, including some preventive medications covered at 100%. 

 

The plans differ in what you will pay annually for your health benefits, as well as what you will pay when you receive care.

Connected Care High Deductible Health Plan (HDHP)
Your Costs:

The 2024 annual paycheck deduction is zero. 

How You’ll Pay for Services When You Need Them: 

First, you pay your annual deductible (the annual out-of-pocket amount you pay before the plan begins to pay). Once you have met your deductible, you pay a coinsurance of 5% when in-network and 40% when out-of-network*. You’re protected by the out-of-pocket maximum. This is the maximum annual amount you would pay out-of-pocket for in-network care.

Health Savings Account (HSA): If you select the Connected Care HDHP, you can enroll in an HSA with Fidelity to help you pay for healthcare expenses (deductibles, coinsurance, and copayments) with pre-tax dollars.

You will have a paycheck deduction for this plan.

How You’ll Pay for Services When You Need Them: 

For in-network care, you pay a copayment when you access care.  For out-of-network care, you are required to pay a deductible. After you the meet the deductible, you pay 40 percent coinsurance and reimbursement limits apply. You’re protected by the out-of-pocket maximum. This is the maximum annual amount you would pay out-of-pocket for in-network care.

*$0 copayment per primary provider visit at the Intel Health for Life Center

Connected Care Pharmacy Benefits

Both Connected Care plan options include comprehensive prescription drug benefits. Some prescriptions are even covered at 100 percent. You can receive your prescription drugs either at a retail pharmacy location or through the OptumRx Mail Order program. You may purchase up to a 90-day supply at your retail pharmacy or through mail order.
